The Primary Health Care Transition Fund operational grants were developed to support provinces and territories in their efforts to enhance the primary health care system, specifically for transitional costs associated with introducing new approaches to primary health care delivery. The initiative was focused on interdisciplinary teams linked to research, evaluation, demonstration, mental health and rehabilitation demonstration, accreditation and leadership training projects. The Ontario College of Family Physicians was the grant holder of nine of these projects listed below. In addition, the members of our Executive Committee especially our CEO Jan Kasperski was on the Steering Committee of more than 30 of these projects. None of the projects could have completed without the engagement and support of our members. We are grateful to the Ministry of Health and Long Term Care for the funding to undertake these projects and for the patience, support and guidance that the MOHLTC staff provided during the execution of the projects. We have high regard for their leadership and their willingness to work so cooperatively with the OCFP and our team of researchers. Lastly, we are in awe of the researchers who worked so diligently on all of these projects and grateful to the members of the Steering Committees for their wisdom, guidance, support and hard work.
