Jan Kasperski Leadership Award Nomination Form

The Jan Kasperski Leadership Award was created in 2008 and is presented to an outstanding leader who best exemplifies exceptional leadership in the pursuit of excellence in family medicine in Ontario. It is created in honor of Jan Kasperski, Chief Executive Officer of the Ontario College of Family Physicians to celebrate 10 years of dedication to our College by demonstrating stellar qualities in the five fundamental leadership practices according to Kouzes and Posner1, 2, 3. In addition to Jan, past winners of the Award include Drs. Walter Rosser and Val Rachlis.


Person you would like to nominate
In preparing your nomination package:
  1. Describe the nominee and how he/she demonstrates the fundamentals of leadership identified by Kouzes and Posner (see attached).
  2. Provide evidence of the significant changes in the healthcare system locally, provincially or nationally or in the academic environment and/or activities or projects that were achieved as a result of the nominee’s leadership excellence.
  3. Provide testimonies from the public, peer organizations, etc. that demonstrate that the nominee is recognized for his/her leadership excellence.
